Blinded
Cannot see. Auto-fail sight-based checks. Attack rolls have disadvantage. Attacks against have advantage.
Charmed
Cannot attack charmer. Charmer has advantage on social interaction checks.
Deafened
Cannot hear. Auto-fail hearing-based checks.
Frightened
Disadvantage on ability checks and attack rolls while source is in sight. Cannot move closer to source.
Grappled
Speed becomes 0. Cannot benefit from speed bonuses. Ends if grappler is incapacitated.
Incapacitated
Cannot take actions or reactions.
Invisible
Impossible to see without special sense. Heavily obscured. Attack rolls have advantage. Attacks against have disadvantage.
Paralyzed
Incapacitated. Cannot move or speak. Auto-fail STR and DEX saves. Attacks have advantage. Hits within 5ft are crits.
Petrified
Transformed to stone. Incapacitated. Cannot move or speak. Unaware of surroundings. Resistant to all damage. Immune to poison/disease.
Poisoned
Disadvantage on attack rolls and ability checks.
Prone
Disadvantage on attack rolls. Attacks within 5ft have advantage. Ranged attacks have disadvantage. Move costs extra.
Restrained
Speed becomes 0. Attack rolls have disadvantage. DEX saves have disadvantage. Attacks against have advantage.
Stunned
Incapacitated. Cannot move. Can only speak falteringly. Auto-fail STR and DEX saves. Attacks have advantage.
Unconscious
Incapacitated. Cannot move or speak. Unaware. Drop everything held. Fall prone. Auto-fail STR and DEX saves. Attacks have advantage. Hits within 5ft are crits.
Exhaustion
Multiple levels of fatigue. Check rules for specific effects at each level (1-6).
